How do I upload my referral and Mental Health Treatment Plan?
Upload your GP referral and MHTP on your Documents page. PDF, JPEG, PNG and HEIC files up to 50MB are accepted.
You can upload your documents while you book, or later from your account.
- Log in.
- Select Documents.
- Next to GP referral or Mental Health Treatment Plan, select Upload and choose your file. You can also drag a file onto the row.
We accept PDF, JPEG, PNG, HEIC and HEIF files up to 50MB. A photo taken on your phone is fine. If your letter has several pages, a PDF scan with all the pages is best.
How do I know it worked?
You'll see Uploaded and sent to the clinic for review. The document then shows as Checking your referral while our team reviews it. You don't need to do anything else.
Please upload your referral at least 24 hours before your appointment. Otherwise you may be charged the private session fee.
The upload isn't working
- "This file is too large": the file is over 50MB. Take a photo of each page instead, or save the scan at a lower quality.
- "Invalid file type": save the document as a PDF or take a photo of it.
- "Network error" or the upload times out: check your internet connection and try again, or try another browser.
If it still won't upload, email your documents to [email protected] with your full name and date of birth. You'll get an automatic reply to confirm we've received them.
