For insurers, lawyers and case managers: how do I request records or a report?
Email [email protected] with the patient's written consent. Reports and records requests aren't covered by Medicare and may have a fee.
We can provide records, reports and treatment updates when the patient has given written consent.
How to make a request
Email [email protected] with:
- the patient's full name and date of birth
- what you need, for example a treatment summary, report or copy of records
- the patient's signed consent or authority to release information
- your deadline, if there is one (for example a court or subpoena date)
- your billing details, if you're paying for the report
We pass your request to the patient's psychologist. Reports and letters aren't covered by Medicare, so a fee may apply. We'll let you know the fee before any work starts. See what services may incur additional fees?

Please send these to [email protected] as early as possible and state the production date clearly in your email.
