What is the intake form link you sent me?
After we receive your GP referral, we send you a link to finish signing up and book your first appointment.
When your GP sends us your referral, our team checks it. Then we send you an SMS that says Please complete your intake form.
The link takes you to our booking page. Completing your booking is your intake. You'll:
- choose a psychologist and a time
- confirm your details and Medicare card
- check your documents (your referral will already be with us)
If the link opens the booking page, that's expected.
If you've already booked, you don't need to do anything else. Your psychologist may send you questionnaires to complete before your first session, such as a DASS-21.
